New Forms Available | March 2024

We have made some changes to the way we accept instructions for making partial or full withdrawals from a Wealth account, and adding a managed portfolio to a Wealth account for the first time. These changes only relate to Mason Stevens Global Investment Service and do not impact Mason Stevens Super.

Withdrawal Form

Instructions to make a partial withdrawal, full withdrawal and closure of an account, must be provided using the new Global Investment Service Withdrawal Form. The form can also be used to transfer cash between your client's Wealth accounts held with Mason Stevens. 

This Withdrawal Form replaces the Account Closure Form and the Internal Cash Transfer Instruction and Indemnity Form. Instructions provided using those two forms will not be accepted from 1 April 2024.

Managed Portfolio Subscription Form

This form replaces the Investment Allocation Form and should be used when you are adding a managed portfolio to a Wealth account for the first time. 

The new Managed Portfolio Subscription Form contains the Managed Discretionary Account (MDA) Service Terms which the client is required to acknowledge before they can invest using a managed portfolio.

You can upload the signed form online through the Add Portfolio functionality.

Please note that the Investment Allocation Form will not no longer be accepted by the platform from 1 April 2024.
