How Do I Apply For A Benefit Payment Request?

You will need to complete the Benefit Payment Request Financial Hardship form.

1. Navigate to the vertical menu. Click Literature Library.

2. Under the Forms heading, you will find the Benefit Payment Request Financial Hardship form.

Certified ID and bank statement will be required- no third-party payments are allowed.
